As we age, it’s easy to dismiss aches and pains as just “getting older.” But what if that nagging shoulder pain and morning stiffness are signs of something more?

There’s a condition called Polymyalgia Rheumatica (PMR) that often flies under the radar—and it mostly affects people over 50. If left untreated, it can seriously limit your ability to move and enjoy life. But the good news? Catching it early can make a big difference.

What is Polymyalgia Rheumatica?

Polymyalgia Rheumatica (PMR) is an inflammatory disorder that causes widespread muscle pain and stiffness, especially in the shoulders and hips. It’s often mistaken for arthritis or simple muscle strain, which means people sometimes suffer for months or even years before getting the right diagnosis.

Let’s take a closer look at the warning signs.


1. Shoulder Pain That Won’t Go Away

For most people, PMR starts with aching shoulders—often on both sides—and it can get worse at night. You might notice:

  • Trouble lifting your arms (even putting on a shirt becomes a struggle).

  • A dull, deep ache in your upper arms.

  • Pain that spreads to your upper back or chest.

At first, it might feel like you just pulled a muscle—but it doesn’t go away with rest.


2. Aching Hips, Thighs, and Neck

PMR doesn’t stop at the shoulders. It can also cause pain in other major muscle groups:

  • Hips and thighs – Walking, standing up, or climbing stairs becomes tough.

  • Neck – Turning your head feels stiff and painful.

  • Upper arms and buttocks – Lifting or sitting for too long becomes uncomfortable.

This pain isn’t relieved by rest and may actually feel worse after being still for a while.


3. Morning Stiffness That Lasts for Hours

This is one of the clearest signs of PMR.

  • You wake up feeling like your muscles are “locked.”

  • It takes over an hour just to start moving normally.

  • Even after loosening up, the stiffness returns if you sit too long.

People often describe mornings as their most difficult time of day, and simple tasks—like brushing your hair or getting dressed—become frustrating.


4. Losing Range of Motion

Because of the pain and stiffness, people with PMR often stop moving certain ways. Over time, this leads to:

  • Limited ability to raise arms or reach overhead.

  • Difficulty bending, twisting, or walking.

  • A general feeling of being “stuck” or unsteady.

Avoiding movement actually makes things worse, which is why early treatment matters.


5. Pain in Knees, Elbows, or Wrists

Although PMR mainly affects larger muscle areas, some people also feel pain in smaller joints like:

  • Knees – Walking becomes harder.

  • Wrists – Holding or typing is uncomfortable.

  • Elbows – Bending arms can feel strained.

Unlike arthritis, PMR doesn’t typically cause swelling or permanent joint damage—but the pain can still interfere with daily life.


6. Other Subtle Clues

PMR doesn’t just affect your muscles. It can come with other symptoms, including:

  • Fatigue or feeling “wiped out.”

  • Low fever or flu-like symptoms.

  • Unintentional weight loss.

  • Feeling low or depressed, often from dealing with pain and reduced mobility.


When to See a Doctor

If you or someone you love is over 50 and experiencing persistent stiffness, fatigue, or unexplained muscle pain, don’t ignore it. PMR responds very well to treatment—usually low doses of corticosteroids like prednisone.

Most people feel relief within days of starting treatment. Without it, the condition can make everyday life difficult and even lead to long-term disability.
